Please download, complete, and fax (West Hills 747-900-6114, Calabasas 747-226-0919) the Intake form that best fits your insurance type (Medicare, Private, or Work Comp). You may also scan and email the form to us at contact@fmphysicaltherapy.com. This form will allow us to verify your insurance benefits prior to your initial evaluation.
